Sciatica

Sciatica is plaguing more and more people in Wrocław, Poland as well as other cities where people are spending so much time in front of a computer for work.​

Statistics show that 40% of the population will suffer from radiating pain down their leg at least once in their lifetime.
​
Sciatica is a general term, and many conditions can be the culprit for causing your pain that radiates down the leg.
There are many causes of Sciatica, below are a few of the most common:
Lumbar Disc Herniations
Between each vertebra of the spine sits an Intervertebral disc. These discs act as a shock absorber (similar to shocks in a car) to protect the bones of the spine from pushing against each other. They also allow movement to occur. It is comprised of two parts, an outer layer called the annulus fibrosis, and an inner layer called the nucleus pulposus.
An intervertebral disc herniation can occur traumatically, all at once, say a deadlifting injury at the gym, or it can happen, and most commonly does happen, slowly from prolonged sitting. Over time the nucleus pulposus delaminates the layers of the annulus fibrosus and it is the nucleus pulposus that herniates out of place.
This leaked out material can either put direct pressure on the nerve root or spinal cord, causing a variety of symptoms including but not limited to: pain radiating down the leg, parasthesias, numbness and in some cases even weakness in the leg.
Predisposing Factors
A co-morbid factor of this condition is often a desk job that involves sitting for most of the day. This type of patient usually has poor sitting posture, which can lead to a weakening of your spinal erectors and destabilize the spine, contributing to the condition.


Piriformis Syndrome​

The piriformis muscle is an external rotator of the hip as well as a stabilizer of the pelvis.
​
In about 15% of the population of Wrocław (and the rest of the world of course) the Sciatic nerve runs directly through the piriformis muscle. This means that if the piriformis is tight, it can compress the sciatic nerve leading to Sciatica type symptoms.​

In this case, treating the spine would bring some relief but would never fix the problem. In cases of Piriformis syndrome, the hip and pelvis need to be released and the patient needs to train better stability in their hip.
Symptoms of Piriformis Syndrome:

Burning pain, numbness or parasthesias down the leg that gets better with stretching the hip

Feeling of stiffness and limited mobility in the hip

Difficulty with single leg balance.

Pain worsening when sitting, driving a car or getting up.
